Make sure you have all the required information at hand. To correctly complete the form, you will need:
Club details (name, code, email, tax address, VAT/Tax ID)
Contacts of the team manager attending the tournament and the defibrillator (AED) officer
Number of teams registered in each category (U13, U14, U16, etc.)
For each team:
Number of players, staff members, and chaperones
Quantity of t-shirts by size (XS, S, M, L, XL, XXL)
Any food intolerances (with full name)
Name of the scorekeeper
Optional: club logo and team photo to attach
Proof of payment of the registration fee (can also be sent later)
